Background: Editorial peer review is universally used but little studied. We examined the relationship between external reviewers' recommendations and the editorial outcome of manuscripts undergoing external peer-review at the Journal of General Internal Medicine. Methodology/Principal Findings: We examined reviewer recommendations and editors' decisions at JGIM between 2004 and 2008. For manuscripts undergoing peer review, we calculated chance-corrected agreement among reviewers on recommendations to reject versus accept or revise. Using mixed effects logistic regression models, we estimated intra-class correlation coefficients at the (...) reviewer and manuscript level. Finally, we examined the probability of rejection in relation to reviewer agreement and disagreement. The 2264 manuscripts sent for external review during the study period received 5881 reviews provided by 2916 reviewers; 28% of reviews recommended rejection. Chance corrected agreement on rejection among reviewers was 0.11. In mixed effects models adjusting for study year and manuscript type, the reviewer-level ICC was 0.23 and the manuscript-level ICC was 0.17. The editors' overall rejection rate was 48%: 88% when all reviewers for a manuscript agreed on rejection and 20% when all reviewers agreed that the manuscript should not be rejected. Conclusions/Significance: Reviewers at JGIM agreed on recommendations to reject vs. accept/revise at levels barely beyond chance, yet editors placed considerable weight on reviewers' recommendations. Efforts are needed to improve the reliability of the peer-review process while helping editors understand the limitations of reviewers' recommendations. © 2010 Kravitz et al. (shrink)

Medieval theology had an important influence on later philosophy which is visible in the empiricisms of Russell, Carnap, and Quine. Other thinkers, including McDowell, Kripke, and Dennett, show how we can overcome the distorting effects of that theological ecosystem on our accounts of the nature of reality and our relationship to it. In a different philosophical tradition, Hegel uses a secularized version of Christianity to argue for a kind of human knowledge that overcomes the influences of late-medieval voluntarism, and some (...) twentieth-century thinkers, including Benjamin and Derrida, instead defend a Jewish-influenced notion of the religious sublime. It was a bad day. First I presented my idea about a Central America protest to the faculty committee, but the committee played ping-pong with the idea until it was crushed. Then I met Robinson, who has somehow been able to present his theory of action in a serious journal. But the theory is a house of cards, and once his critics rattle the table a bit, the theory will come crashing down. And his book on the history of philosophy, (...) just published, is a cheap TV dinner. He does not understand how to study the history of thought. It is necessary to dig into hidden layers of thought, to uncover earlier strata, to map the subterranean patterns, and then to explain why upheavals take place when they do.The above paragraph contains a number of metaphorical sentences. Now all of the following are, I believe, facts about, or relevant to, those sentences. They can be true or false, and our behavior towards them shows that we take them as candidates for truth. I will be expected to offer evidence in support of their truth, and Robinson's supporters on the faculty, if they hear me state the sentences about him, will accuse me of having made false claims. It is generally clear, in regard to the metaphorical sentences under consideration, what would count as evidence for them and against them. (shrink)

This paper examines Angela Davis’s 1969 Lectures on Liberation and her critique of Jean-Paul Sartre’s views regarding freedom and enslaved agency. Across four sections, the paper etches out Davis’s response to what she calls Sartre’s ‘notorious statement’ through her own existential reading of Frederick Douglass’s resistance to chattel slavery. Instead of interpreting Davis’s existential insights through the work of Sartre or other Western continental philosophers, the paper engages Lewis Gordon, George Yancy, Frank Kirkland, and LaRose Parris to develop an (...) alternative frame for assessing Davis’s existential thinking. Embracing a diverse lineage of existential philosophy, the paper argues for Black-centered approaches to existential philosophy that resonate with, but are not reducible or indebted to, European existentialism. Heuser, Eller and Byrne provide important descriptive ethics data about how physicians counsel women on the clinical management of pregnancies complicated by severe fetal anomalies. The authors present an account of what such counselling ought to be based on, the ethical concept of the fetus as a patient and the professional responsibility model of obstetric ethics. When there is certainty about the diagnosis and either a very high probability of either death as the outcome of the anomaly or survival (...) with severe and irreversible deficit of cognitive developmental capacity as a result of the anomaly diagnosed, the pregnant woman should be offered the alternatives of aggressive and non-aggressive obstetric management and induced abortion before viability. It is also ethically permissible to offer feticide followed by termination of pregnancy after viability in such cases. This ethically justified approach will reduce the variation in the actual practices of specialists in maternal–fetal medicine described by Heuser, Eller and Byrne. (shrink)

Thomas GoodnightThere are moments in history that appear to be alive with emancipatory possibilities. Such were the years moving toward the end of the long twentieth century. In spring 1989, students protested the communist regime in China; the Tiananmen Square massacre initiated an episode of opposition and commenced China’s modern journey toward global reengagement. Revolutions (...) in Europe that autumn were more peaceful. On August 23 two million Estonian, Latvian, and Lithuanian citizens held hands and announced the Baltic Way as a departure from Soviet rule. Transitions rapidly followed in Hungary, Bulgaria, and East Germany, culminating in the Velvet Revolution where on November 17 student protests were beaten back by police in Prague only to be followed three days later by an assembly of 200,000 demanding the transfer of power from the Communist Party. On December 3, George Bush and Mikhail Gorbachev jointly announced an imminent end to the fifty-year-old Cold War. During this time of hopeful transition, two scholars were at work refiguring the philosophical mission of the Frankfurt school, transforming critique from its early totalizing posture into the critical study of communication.Thomas B. Farrell and Jürgen Habermas were trained in different traditions but adjacent disciplines, communication and sociology. Their works [End Page 421] share a commitment to philosophical inquiry directed toward critique and understanding the cultural, social, and linguistic constitution of communication. As a rhetorician and as a philosopher, these scholars share an evident passion for scholarly argument and for engagement with the controversies of public culture. In different ways, both were at work during the portentous years of transition from the Cold War to new democracies and incipient globalization. Each constructed a guarded program of hope in the possibilities of communicative reflection and reasoning. In the post-World War II era, such a broad, positive vision took long to develop. After the brutal realities of the Holocaust became publicly known in the late 1940s, “the generous faith in rationality and the possibilities of human progress which underlay much of earlier historical thought seems discredited today” (Katznelson 2003, 2). The Enlightenment possibilities for reason had been invested in the technical efficiency of modern institutions that produced the capacity to kill systematically without constraints. Ira Katznelson points to a generation of scholars—including individuals at Chicago, Columbia, the New School, and others—who began in the 1950s to engage in a project that would “shade the Enlightenment’s philosophical anthropology, thicken its defenses against evil, deepen its capacity to reason, and restore” an Enlightenment more critical and less susceptible to co-optation by the excesses of nationalism, ideology, and evil (2003, 2). The work of Farrell and Habermas belongs to the extended post-Holocaust moment, where the possibilities of reflective reason in ethical and moral deliberations were set carefully on a road to personal and collective emancipation.Max Horkheimer saw in the modern, postwar welfare state little reason for optimism. Writing in 1946 at the beginning of the Cold War, he concluded: “Language has been reduced to just another tool in the gigantic apparatus of production in modern society” (1974, 22). “Salestalk” by “the surrogates of culture” creates an “illusory triumph of democratic progress that consumes the intellectual substance on which democracy has lived” (Horkheimer 1974, 31). Scientifically engineered propaganda renders public opinion as “a substitute for reason.” With such tools, the forces of barbarism are always waiting in the wings to seize power. Early on, Habermas shared these views. His thesis, in The Structural Transformation of the Public Sphere (1989a), tracks the opening possibilities of communicative reasoning in the liminal spaces of eighteenth-century coffeehouses where frank talk replaced hierarchically driven politeness, the development of these talks into the practices of deliberative democracy, and the decline of deliberative democracy with the union of the state and mass media in producing spectacles in [End Page 422] support of rituals of legitimation. Habermas broke with Horkheimer over the dissertation, however. A thesis of decline, by itself, leaves open only the route of totalizing critique. ‘What it is like’ talk (‘WIL-talk’) — the use of phrases such as ‘what it is like’ — is ubiquitous in discussions of phenomenal consciousness. It is used to define, make claims about, and to offer arguments concerning consciousness. But what this talk means is unclear, as is how it means what it does: how, by putting these words in this order, we communicate something about consciousness. Without a good account of WIL-talk, we cannot be sure this talk sheds light, (...) rather than casts shadows, on our investigations of consciousness. The popular technical account of WIL-talk (see e.g. Lewis, 1995, and Kim, 1998) holds that WIL-talk involves technical terms — terms which look like everyday words but have a distinct meaning — introduced by philosophers. I argue that this account is incorrect by showing that the alleged technical terms were not introduced by philosophers, and that these terms do not have a technical meaning. (shrink)

Is it rational to be moral? How do rationality and morality fit together with being human? These questions are at the heart of David Schmidtz's exploration of the connections between rationality and morality. This inquiry leads into both metaethics and rational choice theory, as Schmidtz develops conceptions of what it is to be moral and what it is to be rational. He defends a fairly expansive conception of rational choice, considering how ends as well as means can be rationally chosen (...) and explaining the role of self-imposed constraints in a rational life plan. His moral theory is dualistic, ranging over social structure as well as personal conduct and building both individual and collective rationality into its rules of recognition for morals. To the "why be moral" question, Schmidtz responds that being moral is rational, but he does not assume we have reasons to be rational. Instead, Schmidtz argues that being moral is rational in a particular way and that beings like us in situations like ours have reasons to be rational in just that way. This approach allows him to identify decisive reasons to be moral; at the same time, it explains why immorality is as prevalent as it is. This book thus offers a set of interesting and realistic conclusions about how morality fits into the lives of humanly rational agents operating in an institutional context like our own. (shrink)

Various claims have been made concerning the role of narrative in grief. In this paper, we emphasize the need for a discerning approach, which acknowledges that narratives of different kinds relate to grief in different ways. We focus specifically on the positive contributions that narrative can make to sustaining, restoring and revising a sense of who one is. We argue that, although it is right to suggest that narratives provide structure and coherence, they also play a complementary role in disrupting (...) established structure and opening up new possibilities. We add that both of these roles point to the importance of interpersonal, social and cultural factors in shaping the trajectory of grief. We conclude by briefly considering the implications for distinguishing between typical and pathological forms of grief. (shrink)

The only one of these to survive is his inaugural oration on the history and utility (...) of the mathematical arts.1 Regiomontanus tells his audience that the purpose of the oration is torelate first the origin of our arts, and among which nations they first began to be cultivated, in what way they were at last translated from various foreign tongues into Latin, which of our ancestors were famed in these disciplines, and to whom in our lifetimes recognition should be granted.2To this end, he offers a history of the quadrivial arts (arithmetic, geometry, music, and astronomy) and other important mathematical disciplines from [End Page 41] antiquity to his own time, praises their utility, and exhorts his audience to revive the languishing study of mathematics at Padua. Astrology, a discipline with which Regiomontanus himself was closely associated, is singled out for particular praise.Traditionally, Regiomontanus's Padua oration has been seen through the lens of its rather obvious humanism. In particular, the Padua oration has come to be understood as the rhetorical embodiment of the fifteenth-and-sixteenth-century revival of ancient (Greek) mathematics. Just as this revival is inextricable from the rise of humanism, so Regiomontanus has come to be seen as an exemplar of humanist mathematics.3 It is the aim of this paper to examine the Padua oration in the context both of contemporary humanist rhetoric and of Regiomontanus's own intellectual background in order to argue that, while the oration is stylistically consistent with humanist norms, the vision of mathematics presented in it is also deeply grounded in the university mathematical curriculum and in Regiomontanus's own reading of mathematical texts.Regiomontanus was educated primarily at the University of Vienna (he was also briefly at the University of Leipzig), where he enrolled in 1450, completed his baccalaureate in 1452 and became a master in 1457.4 He remained at Vienna until 1461, when the death of his friend and teacher Georg Peurbach prompted him to travel to Italy with his patron, Cardinal Bessarion.5 In Regiomontanus's day Vienna was probably the most important of the German universities, rivaled only by Prague, whose prestige had declined after it was stripped of its theology faculty in the aftermath of the Hussite Wars.6 The curriculum at the University of Vienna was modeled [End Page 42] after that of Paris, and Parisian scholars played a major role both in its founding in 1365 and in its re-establishment (this time with a theology faculty) in 1384.7Most importantly for the purposes of this paper, the Viennese mathematical curriculum of Regiomontanus's day included all of the traditional authorities taught at Paris in the fourteenth century. For arithmetic and algebra, various "algorisms" (prose or poetry instructions for carrying out arithmetical operations that often also included a small amount of number theory) were the basic texts, supplemented in the fourteenth century by the Quadripartitum numerorum of Jean de Murs. Jordanus de Nemorare's De numeris datis and al-Khwarizmi's Algebra were common sources for those engaged in more advanced studies (i.e., they were not normally the subject of ordinary lectures, but were readily available to interested students, and would perhaps have been the subject of occasional extraordinary lectures). Euclid's Elements, supplemented by the commentaries of Pappus and Campanus, was the central text for geometry, and a number of medieval texts on practical and speculative geometry were in circulation as well. For astronomy, the Sphere of Sacrobosco and the Theorica planetarum were the most commonly used teaching texts, sometimes supplemented by al-Farghani's Elements of Astronomy (the subject of Regiomontanus's Padua lectures). The human body has come to be seen as forever susceptible to both external and internal hazards, which in many circumstances require immediate, heroic, and expensive intervention. In response to this, there has been a shift from a treatment‐based healthcare model to one of prevention wherein nurses play an integral role by identifying and assessing risks for individuals, communities, and populations. This paper uses Deborah Lupton’s outline of the spectrum of risk and applies the theoretical works of Foucault and Plato (...) to demonstrate the means by which nurses maintain social order by identifying and counselling risk takers. It also utilizes the work of Deleuze and Guattari to illustrate how Plato’s framework for creating social order through the creation of the good citizen can be viewed as a micro‐fascist system, which has been adopted wholeheartedly by preventative health professionals. The goal of this paper is to present an alternate understanding of risk to provide nurses and other healthcare professionals with a non‐traditional appreciation of certain aspects of their practice as researchers and clinicians.
